On your first visit to our office it is helpful to arrive several minutes before your appointment in order to register and be entered into our computer system. On the top of this page is a link to the Patient Identification Form which you may wish to print and complete at your convenience. The few minutes it takes to do this will save you time when you arrive at our office. Accurate information is essential so please be knowledgeable about your medical and vision insurance and bring the appropriate identification cards. Be certain to inform us of any changes in your insurance, address, telephone and email so that we can optimize your benefits. Your email address is very helpful for notification of appointments and alerts. We do not share this information with anyone.
Let us know in advance if you are an existing contact lens wearer or want to explore the possibility of wearing contacts so that we can coordinate your eye exam with the appropriate contact lens evaluation. Since we are ophthalmologists your medical history is particularly important. If you have a printed history and a list of current medications please bring it with you. Alternatively you may wish to print the History Form from the link on the top of this page to enter this information.
A medical eye examination normally (but not always) involves dilation of the pupils and the effects may persist for several hours. If it necessary to examine your retina for medical conditions such as Diabetes, dilation is often essential. We will provide you with temporary sunglasses which will help reduce glare but if you are “far sighted” not only will your close vision be blurred but your distance vision may be affected as well. Although it is usually unnecessary, you may wish to bring a driver, particularly if you have experienced difficulty with dilation on previous exams.

We are providers for most major insurance carriers and will automatically bill for services rendered. If you have a co-payment or other non covered service it will be collected when you register and you will receive a receipt. Fees for cosmetic and LASIK surgery are collectible in advance but terms can be arranged. We accept cash, check , VISA or Mastercard for all transactions.
