New patients will be asked to fill out Patient Information and Medical History forms on their first visit to the Eye Medical Clinic. You can save some time by downloading and printing these forms so you have the opportunity to complete them beforehand.
To view and print the forms you will need Adobe® Acrobat Reader®. You can download it from www.adobe.com.
To use a form click on the form name. You may be asked if you want to save or open the form. Click open. When the form is displayed select Print from the File menu to open the printer dialog box.
